Feds Find Culture Of 'Complacency' At Tesoro Before Deadly Refinery Fire

A culture of complacency at Tesoro’s Anacortes refinery led to the deadly fireball that claimed the lives of seven workers in 2010, according to federal investigators who've spent almost four years examining the causes.

The equipment that exploded in the early hours of April 2, 2010 had developed leaks that the company knew about. The carbon steel tubing of the equipment had been weakened over time by hydrogen and that had caused cracks.

Proposed rail-to-ship crude oil terminal biggest yet in region

The proposed Tesoro/Savage crude oil terminal would be built on this rail loop at the Port of Vancouver, Washington on the Columbia River.

Oil refiner Tesoro and a terminal operating company named Savage detailed plans Thursday for the biggest crude oil shipping terminal to be proposed in the Northwest.  

The proposed terminal, which would be located on the Columbia River at the Port of Vancouver in Washington, would receive crude by rail from oil fields in North Dakota and the like. The oil would then be transferred onto oceangoing tankers for delivery to West Coast refineries.

Oil spill scare off coast of Port Angeles shows readiness pays

A small oil spill this week in Port Angeles turned out to be a lot less severe than originally feared.  Clean-up crews are still working to get sticky residue out of the the harbor. But the incident shows how preparedness can pay off.

Washington state put laws in place six years ago that did just what they were intended to do early Wednesday morning, according to the Department of Ecology.

Suit filed over deaths at Tesoro Refinery

The families of six Tesoro Corporation refinery workers killed in an explosion at an Anacortes site last April have filed a wrongful-death suit against the company Wednesday. 

The Seattle Times' Jim Brunner reports the suit alleges Tesoro was negligent in addressing the conditions that resulted in the blast, which killed seven people. In addition to the six families, a contractor injured in the explosion has also joined the suit.
